Waiver & Release of Claims

Please read this form carefully and be aware of registering yourself for participating in any adult event at the Kamloops Curling Club. You will be waiving & releasing all claims to injuries you might sustain arising out of the Kamloops Curling Club programs. By signing below, I recognize & acknowledge that there are certain risks of physical injury to participate in curling and agree to assume the full risk if any such injuries, damage, or loss regardless of severity, which I may sustain as a result of participating in any activities connected or associated with any Kamloops Curling Club programs. I waive and relinquish all claims that I, my insurer, may have against the Kamloops Curling Club and its officers, agents, & employees from an & all claims from injuries, damages or loss which I may have, or which may accrue to me on account of my participation in curling or events held at the Kamloops curling club.
